At-Home Health care is a hospital based, Medicare certified home health agency whose priority is to care for our patients with compassion and understanding.  Our team of health care professionals, under the direction of your Doctor, will provide the highest quality of care for you in the comfort of your home.  We can teach you to cope and thrive with certain diseases or conditions.  Our team of trained home health professionals can help you understand your prescribed diet and medication needs and provide Physical Therapy and other health care needs you may have.

How Can I Begin Receiving Home Health?

Sparta Community Hospital’s At-Home Health Care serves those people who are essentially homebound, in need of skilled care and reside within a 30-mile radius of Sparta Community Hospital.  Our service areas include Randolph County as well as portions of Perry, St. Clair, Washington and Jackson Counties.

Family, friends, physicians and hospitals can make referrals or other health agencies however you will need a physicians order for care.  If you are a hospital patient, your physician can order home health services to begin when you go home.
