Social Services
The Sparta Community Hospital Department of Social Services offers services designed to help both patients and families cope with the difficulties associated with disease and illness. The Social Service department works with the other hospital departments and community agencies to create a network of services. The Social Services department’s job starts the day you are admitted into the hospital when our social services department begins to evaluate what services or assistance may be needed upon discharge. Service offered include those listed below:
- Patient Evaluation upon admission and discharge
- Referral to Home Health
- Referral to Private Duty Home Maker
- Meals delivered to the home (community based services)
- Referral for Short or Long Term care (nursing home)
Other services offered are for community members, groups or employers in the Sparta Community Hospital District’s service area.
