Department of Social Services

The Sparta Community Hospital Department of Social Services offers services designed to help both patients and families cope with the difficulties associated with disease and illness.  The Director of Social Services, Lynn Marie, has a wide range of experience in the Social Service arena. The Social Service department works with the other hospital departments and community agencies to create a network of services.  The Social Services department’s job starts the day you are admitted into the hospital when our social services department begins to evaluate what services or assistance may be needed upon discharge.  Service offered include those listed below:
  • Patient Evaluation upon admission and discharge
  • Referral to Home Health
  • Referral to Private Duty Home Maker
  • Meals delivered to the home (community based services)
  •  Referral for Short or Long Term care (nursing home)

 

Other services offered are for community members, groups or employers in the Sparta Community Hospital District’s service area.

  • Information on Community Resources

The Social Service Department also offers courses to community groups on:

  • Abuse and Neglect (Child or Elderly)

  • Stress Management

  • Referrals for financial need (such as Kid Care, Social Security Disability or Department of Public Aid)

The Social Service Department can also help you with documents such as Health Care Power of Attorney and Advanced Directives.  You may print the forms below and complete them at your  leisure.  Please contact Lynn Marie Tadda at 618-443-2177 ext 1409 with any questions you may have.  Please bring a copy of each when you next visit the hospital and ask that they be added to your chart.
