TO BECOME A MEMBER Complete the enclosed registration card and return it and your ten dollar membership fee to Sparta Community Hospital VIP Program. You will receive your free card within a week. Additional forms can be obtained by call (618)443-2177. PATIENT ADVOCATE Members receive assistance with questions and concerns related to health insurance and processing of health insurance claim forms. HEALTH SEMINARS A variety of educational programs are offered to members throughout the year. Programs will focus on health and wellness concerns specified to adults 55+. Rremaining programs this year are listed below:
SOCIAL EVENTS VIP members will receive a personal invitation to various social events that will be held throughout the year. NEWSLETTERS A newsletter is mailed quarterly to members. Information on upcoming program, events and new benefit additions are highlighted along with informative articles. EMERGENCY RESPONSE SYSTEM As a VIP member you will receive the first month of the Emergency Response System free. SPECIAL SERVICES WHEN YOU VISIT THE HOSPITAL VIP members receive special privileges such as private room when possible, and free meals will be delivered to your room for a family member (1 per day). In addition, after you are discharged from the hospital, the patient advocate will contact you to see if you have any questions or complaints concerning your hospital stay.
