During Sparta Community Hospital’s monthly leadership team meeting, CEO Joann Emge announced that the Pharmacy Department is the recipient of this month’s Quality Spotlight.
Effective communication about medicine prevents misunderstanding and non-adherence that could lead to serious problems for a patient. Concerned that over half of adverse events after being discharged from a hospital are medication related, the Pharmacy team at Sparta Community Hospital focused efforts on their medication education process for inpatients.
To measure the success of their efforts, the team set a goal to raise their HCAHP Medication about Communication score to 64%. For March, April and May, the Pharmacy team exceeded their goal by scoring 75, 80, and 91% respectively.
Each month, Sparta Community Hospital recognizes one department who has met one of their target’s for three consecutive months. A traveling trophy is awarded and displayed in the wining department for one month.
