ADAMS SPEAKES ON CARE TRANSITIONS

 Sparta Community Hospital’s At-Home Health Care Administrator, Cheryl Adams, was a featured presenter at the Annual Illinois Home Care and Hospice Council Conference on Wednesday, April 5, 2023.   Adams’ presentation ‘Care Transitions – Reducing Rehospitalizations’ shared how utilizing home health agencies as a patient transition through the different levels of care will assist hospitals in reducing rehospitalizations and emergency department visits, thus impacting the agencies stance in Value Based Purchasing.

“All home health clinicals have the ability to provide consistent care and support patients through personalized experiences,” states Adams.  “I learned early on that home health clinicians have the power to be a lifeline for patients in more ways than I could have imagined,” states Adams.  “Patients depend on us, and after decades of life changing experiences in homecare, I can’t think of a better place to fulfill the purpose of what got me into nursing in the first place: caring for people.”
