Take Control Before Diabetes Starts

Small changes today can make a big difference

What if you could stop diabetes before it ever begins?

For many people, that opportunity is closer than they think.

According to national data, 1 in 3 adults has prediabetes—but only about 11% know it . Without lifestyle changes, many of those individuals will go on to develop Type 2 diabetes within just five years.

But here’s the good news: you can change that path.

At Sparta Community Hospital, we believe in giving our community the tools, support, and guidance needed to take control of their health—before problems begin.

A Program Designed to Help You Succeed

Our National Diabetes Prevention Program (NDPP) is a year-long lifestyle change program designed to help you prevent or delay Type 2 diabetes.

This program is built on the same CDC-backed approach used nationwide, helping you make small, manageable changes that add up to big results.

Led by Courtney Johnson, MS, RD, LDN, CDCES, the program focuses on building real-life habits that last—not quick fixes.

You’ll learn how to:

  • Make healthier food choices that fit your life
  • Build physical activity into your routine
  • Manage weight in a sustainable way
  • Stay motivated with support from others on the same journey

This isn’t about perfection. It’s about progress.

What Makes This Program Different

This program goes beyond information—it’s about transformation.

Participants receive:

  • Guidance from a Certified Diabetes Care and Education Specialist
  • Practical, easy-to-follow strategies
  • Support from a group working toward similar goals
  • Tools to create lasting lifestyle changes

After the first 16 weeks of weekly sessions, the program continues with follow-ups to help you stay on track and maintain your progress.

Because real change doesn’t happen overnight—it happens with consistency and support.

Real Goals. Real Impact.

The program is built around achievable, proven goals:

  • Lose 5–7% of body weight
  • Reach 150 minutes of physical activity per week
  • Reduce your risk of developing Type 2 diabetes

These small changes can lead to big results—not just in preventing diabetes, but in improving your overall health, energy, and quality of life.
