Sparta Community Hospital’s At-Home Health Care program has once again been recognized as a 2025 National Best Practice Agency by WellSky, earning the Home Health CAHPS Award of Distinction — a recognition the organization has maintained for more than 15 consecutive years.
Based on a comparative analysis of agencies participating in WellSky’s national Home Health CAHPS database, Sparta Community Hospital’s At-Home Health Care ranked in the top 25% nationally for Overall Satisfaction and Likelihood to Recommend.
Agencies named National Best Practice leaders have demonstrated excellence in one of the most meaningful measures of quality in home health care — the patient experience. Maintaining this distinction for over 15 years reflects a sustained commitment to compassionate care, strong communication, and consistency in performance.

The Home Health Consumer Assessment of Healthcare Providers and Systems (HHCAHPS) survey measures patients’ perceptions of communication, responsiveness, and overall satisfaction with their home health provider. Ranking in the top quartile nationally — especially over a span of more than 15 years — demonstrates not just excellence, but trust built over time.
