The National Committee for Quality Assurance (NCQA) congratulates Sparta Community Hospital’s Quality Healthcare Clinic, Family Health Centre, on their transforming Patient-Centered Medical Home Recognition.

What is a Patient Centered Medical Home?
A patient centered medical home (PCMH) is a quality-driven, clinician-led approach to coordinating and delivering care that puts patients, family members, and personal caregivers at the center of all decisions concerning the patient’s health and wellness. A PCMH provides comprehensive and individualized access to physical health, behavioral health, and supportive community and social services, ensuring patients receive the right care in the right setting at the right time.

Data shows that this proactive approach to patient health accounts for a significant reduction in unnecessary emergency department visits and hospitalizations. These reductions then translate to lower incidences of illness, mortality, and lost days of productivity; and they lead to increased cost savings for the patient and the health care system.

Family Health Centre joins QHC’s Coulterville Medical Clinic, Marissa Medical Clinic, Sparta Medical Office and Steeleville Clinic who also hold PCMH Recognition status.
